Use the Forms app on your instance
Overview.
The Forms application allows you to create forms and share them with users on your instance or with other people using external sharing. Each user can create a form and share it.
Accessing the Forms application.
To access the Forms application, accounts authorized to create forms must click on the application icon at the top left of the interface.
This is where the forms appear. If you don't have any, you can create one by clicking on New Form or Create Form.
When creating a form, you must enter a name and description. Then click on Add a question to display the following options:
Checkboxes: allows you to offer answers with multiple selectable choices.
Radio buttons: allows you to offer answers with a single choice.
Dropdown: allows you to offer answers with a single choice in the form of a list.
File: allows people responding to the form to upload a file.
Short answer: allows you to enter a short text.
Long text: allows you to enter a more complete text.
Date: allows respondents to select a date.
Time: allows respondents to select a time.
You can view the form at any time, even while it is being created, by clicking on the View button at the top right.
As shown in the screenshot, several types of questions can be selected within the form.
Back on the form editing screen, using the Edit button at the top right, you can set options for each question.
These options depend on the type of question.
The order of the questions can also be changed using the arrows on the left-hand side.
Once the form is ready, you can share it with other users or external individuals who do not have an account on the instance. To do so, click on the Share button at the top right.
The sharing panel opens, and the share link allows you to share the form with an external person. This link will be automatically copied to your clipboard, and you can paste it into a message or email to send it to someone who does not have an account on your instance.

The Search for a user, group, or team field allows you to enter a name in order to share the form.
In this example, we are sharing the form with a user named User. Clicking on ... allows you to refine the sharing permissions or delete them.
When the user logs in, the form appears on the left side of the interface, under Shared with you.
They can then fill it out and click Send.
The results are then displayed to those who can access them, along with a figure showing the number of responses.
The results can be detailed, including the option to delete responses.
The Create a spreadsheet button allows you to save a file containing the form results to your drive.
The file can then be opened using the Open Spreadsheet button or by navigating to your files.
